Notice to the reader:
This return must be completed by each official agent of an officially nominated candidate at an election of a member or members to serve in the House of Commons of Canada, unless the appointment of such agent had been terminated prior to completion of the duties in respect of that particular election. The return must be filed within:

• four months after polling day, or,
• if applicable, within four months of the date of publication in the Canada Gazette of the notice of withdrawal, or deemed withdrawal of the writ issued for the electoral district for which the candidate was nominated.
