CPT2 Request for a Ruling as to the Status of a Worker Under the Canada Pension Plan and/or Employment Insurance Act

Form CPT2 has been discontinued. A worker, a payer, or an authorized representative will now use form CPT1 to request a ruling on whether a worker's employment is pensionable or insurable, or both. The new format has been reduced to one page, with instructions on the front and the request on the back.
