Information on ordering 500 copies or more

Use this order form only to order 500 copies or more of forms NR4, T2202A, T4, T4A, T4RIF, T4RSP, T5, T5007 or T5008. To order any other forms, see our Getting forms and publications Web page.

Please note:

• The T2202A, Tuition and Education Amounts Certificate, can also be ordered using this form. Please note that only educational institutions can order it.
• For the 2011 tax year the T4, T4A, T4RIF, T4RSP and NR4 will be obsolete; only the revised 2011 versions of these forms will be valid.

Useful information and instructions for placing orders using this order form:

• If you have any questions about your order, call us at 1-800-959-2221. Refer to your customer number, if you have one, when making enquiries.
• Our printers will begin delivery to CRA offices in November. Since several printers produce the forms, it may not be possible to deliver all of the forms in your order at the same time.
