Make note of each consent you give, so you can cancel them when they are no longer needed.
You can immediately cancel an existing consent by calling us at 1-800-959-8281 or by using the My Account service.
You can also cancel an existing consent by completing parts 1, 5, and 6 of Form T1013, Authorizing or Cancelling a Representative.
Your consent will stay in effect until:
- you cancel it,
- it reaches the expiry date you choose, or
- we are notified of your death.
Your representative may also request by telephone, in person, or in writing, that the consent you have given him of her be cancelled.
Instructions
To accept your request, the following information is required:
- Part 1 - Taxpayer information - see example
- First name
- Last name
- Home or work telephone number
- Account number - at least one account number must be present
- Part 5 - Cancelling one or more existing consents - see example
- Box A - Cancel all consents
- Box B - Cancel the consent given for the individual or business identified below:
- Name of individual and/or business
- RepID (individual), GroupID (group) or Business Number (BN)
- Part 6 - Signature - see example
- The taxpayer or his or her legal representative must sign and date the form. Tick the appropriate box if it is a legal representative. If two or more legal representatives are acting jointly on the taxpayer's behalf, the signature of each legal representative is required.
- The form must be received by the CRA within six months of its signature date. If not, the form will not be processed.
All consents for authorized representatives are automatically cancelled when we are notified of your death.
