This pension reform update corrects postal codes of the taxation centres listed in Exhibit 1 of the Plan Administrators' Past Service Pension Adjustment Guide [T4014(E) Rev. 93].

The remaining information in Exhibit 1 is correct. However, please use the following postal codes to forward T1004 forms or T215 returns to one of the taxation centres listed, when reporting past service pension adjustments:
